I would recommend getting a Hitachi Wand. The intensity of the vibration is better than anything else on the market. Get one from Australia. If you get one from the US you will need a voltage converter. Absolute heaven!!!

The hitachi has a rather bulbous head so perhaps insertion using an attachment would be advisable. Yes they are reasonably quiet. Try googling Hitachi Wand and you will see what I mean.
